Dr. Deborah Richter, chair of Vermont Health Care for All, highlighted the stark inefficiencies within the current framework. She noted that while the U.S. spends double per capita on healthcare compared to other wealthy nations, a massive portion of those funds is diverted to administrative bloat rather than patient care. Since 1970, the number of healthcare administrators in the U.S. has surged by over 4,000%, while the physician workforce grew by only 200%. This bureaucracy, compounded by profit-driven claim denials, has pushed an estimated 98 million American adults to delay or skip medical treatment entirely.

The Push for Transparency and Structural Change

Beyond the call for universal coverage, advocates are focusing on immediate transparency measures. Cynthia Fisher, founder of Patient Rights Advocate, described the current market as a form of extortion where patients are forced to sign “blank checks” for procedures without clear pricing. Despite the PRICE Transparency Act 2.0 and existing federal mandates, many hospitals continue to obscure their costs. Wendell Potter, a former insurance executive turned advocate, argued that the industry’s reliance on the middleman model is increasingly unsustainable. As patients and providers alike push back against the “demoralizing” hoops of prior authorizations and privatization schemes like Medicare Advantage, activists believe the path toward a unified, public system is becoming clearer.
